• Please call and schedule all of your appointments rather than walk-in. We may not be able to accommodate you if you walk in without an appointment.
  • If you have a change in insurance please be sure to bring that information with you to update it with our Registration staff.
  • Please bring all of the medications you (or your family members) are taking with you.  It helps if you bring the actual containers of the medications or a list of the medications including the dosage and the number of times a day the medications are taken. This includes over-the-counter medications.
  • Prior to the visit, write a list of the issues that you have and would like to discuss with your health care provider.
  • Feel free to bring along a family member or friend with you to the clinic.
  • Be sure to ask any questions you may have during or even after the visit, especially if you do not understand what the provider is telling you. You may have some follow up tests or visits that may have to be scheduled. Please confirm this information before leaving the health center.
  • Should you have questions after your visit you can either contact your provider by phone or through My Chart, an online patient portal. If you are not registered for My Chart you can ask your provider how to register or click here for more information.
