Upham’s Corner Health Center offers screening and treatment for infectious diseases, including HIV/AIDS, Hepatitis C, and TB.
Upham’s Corner Health Center provides a wide range of clinical and support services for people living with HIV/AIDS, and other infectious diseases. Our dedicated case managers and nurses work closely with the primary care team to ensure that our clients are connected to care, have adequate housing and food, have access to medications, and have support for other stressors in their lives.
Our medical team offers routine screening and treatment for sexually transmitted infections both in urgent care and primary care. We also offer services dedicated to caring for people with Hepatitis C, including antiviral treatment within the primary care setting, dedicated case management, and support for other social needs. We also offer PrEP (Pre-Exposure Prophylaxis), which is a treatment taken daily to prevent HIV in people who are at high risk.
Our HIV-related services are supported by the Ryan White Care Act Part A and MAI funds. These funds provide support to health center and non-health center clients living with HIV within the Boston Eligible Metropolitan Area.
Patients are supported by an integrated team that includes a primary care provider, nurse case manager, social case manager, pharmacist, counselors, nutritionists, etc.
Please call the health center if you think you have any concerns or would like to be connected to our case management services.
