About Medical Records

Medical records include, but are not limited to a patient’s medical history, test results, immunization record, and office visit notes. The Medical Records Department can provide you with copies of your medical records related to your care at Upham’s.

 

Patient Request Medical Records

If you want to request your records using this website, you need to complete and sign an authorization form.  Please click on the link below to open the authorization form in your language.

Once you complete and sign the ‘Authorization to Release Protected Health Information’ form, please either fax or email the form to Medical Records Department along with a copy of your proof of identification.  Send a FAX at:  617-287-1500 or EMAIL to [email protected].

When we receive the completed and signed ‘Authorization to Release Protected Health Information’ form along with a copy of your proof of identification, it can take up to 7 business days.

Please note that we cannot guarantee the security of emails sent by you.  When we reply to your email, it will be encrypted, which means you need to create a password through Barracuda Email Security to view the response.

If you are enrolled in MyChart, our secure on-line patient portal, you can view certain records.

Non-Patient Requests for Medical Records

If you are a third party requesting a patient’s Medical Records, you must have the patient complete and sign the ‘Authorization to Release Protected Health Information’ form.
