Upham’s Corner Health Center (UCHC) Enrollment Department can help patients of UCHC apply for government sponsored coverage, such as MassHealth, Connector Care, and Health SafetyNet.
The Enrollment staff will assist patients and their families that have no or limited health insurance, obtain the most comprehensive coverage for which they qualify. The Enrollment staff can also assist patients with the MassHealth renewal process in order to maintain coverage.
If patients receive mailings from MassHealth it is very important to respond timely to all renewal notices that have ‘IMPORTANT NOTICE’ on the top of the envelope. Enrollment staff can help you with these notices.
Once you become eligible for MassHealth, many patients need to select a plan, such as Community Care Cooperative (C3), Health Connector Plan (BMC HealthNet Plan and Tufts). The Enrollment staff can help you with the selection/enrollment process and can help you to understand plan benefits and premium payment process.
In order to apply for government sponsored health coverage programs/insurance, please bring the following with you when you come to the department:
- Proof of Massachusetts Residency: Utility Bill OR notarized letter AND driver’s license OR photo ID
- Proof of Income: Two (2) most recent pay stubs or current latest tax information
- Proof of Immigration Status: US Passport, or Permanent Resident Card or Birth Certificate WITH Social Security Card (when available)
If you are not eligible for one of these programs or decide not to apply, UCHC offers a Sliding Fee Discount based on your family size and income.
