Vaccine Eligibility
As of June 30, 2022, anyone ages 6 months+ is eligible for the COVID-19 vaccine.
Upham’s offers the Pfizer and Moderna vaccines for children and the Pfizer, Moderna, and Johnson & Johnson (Janssen) vaccines for ages adults.
Booster Dose Eligibility
Anyone ages 5+ can receive 1 booster after completing their COVID-19 vaccine primary series.
Those who are eligible to receive 2 booster doses:
Consent Forms
Children under the age of 12 must be accompanied by a parent or guardian. For anyone ages 12-17, please come with a parent or guardian and bring the COVID-19 Vaccine Consent Form. If you come alone, please be prepared for us to call a parent or guardian to obtain verbal consent.
