Upham’s Corner Health Center is hosting a COVID-19 vaccination and booster clinic at our main health center, 415 Columbia Road, Dorchester. 

 

Due to patient volume, vaccine and booster appointments must be scheduled in advance. No walk-ins are accepted. We apologize for any inconvenience.

Due to supply shortages, Boston Public Health Commission is no longer providing at-home COVID-19 test kits at community distribution sites. Stay up-to-date with their website.

Vaccine & Booster Eligibility

Youth ages 12+ and adults are eligible for a COVID-19 booster dose.

As of November 10, 2021, anyone age 5+ is eligible for the COVID-19 vaccine.

Upham’s offers the Pfizer vaccine (for 5 years of age and older) and the Moderna and Johnson & Johnson (Janssen) vaccines (for 18 years of age and older).

 

Consent Forms

Children under the age of 12 must be accompanied by a parent or guardian. For anyone ages 12-17, please come with a parent or guardian and bring the COVID-19 Vaccine Consent Form. If you come alone, please be prepared for us to call a parent or guardian to obtain verbal consent.
