Who is eligible for the COVID-19 Vaccine from Upham’s: As of June 9, 2021: anyone age 12+.

For anyone ages 12-17, please come for your vaccine with a parent or guardian and bring the COVID-19 Vaccine Consent Form. If you come alone, please be prepared for us to call a parent or guardian to obtain verbal consent.
