Updated September 28, 2021: The Johnson & Johnson (Janssen) vaccine is not available. Upham’s offers both the Moderna vaccine (for 18 years of age and older) and Pfizer vaccine (for 12 years of age and older).

We are pleased to be able to provide you with trusted vaccine resources and information.

Who is eligible for the COVID-19 Vaccine from Upham’s: As of June 9, 2021: anyone age 12+.

For anyone ages 12-17, please come for your vaccine with a parent or guardian and bring the COVID-19 Vaccine Consent Form. If you come alone, please be prepared for us to call a parent or guardian to obtain verbal consent.
