Jagdeep (Jay) Trivedi brings over twenty years of administrative leadership experience in medical device startups, managed care and alternative payment strategies.  Jay began at Upham’s Corner Health Committee (UCHC) in 2004 as the Operations Director of the Upham’s Elder Service Plan, a Program of All-Inclusive Care for the Elderly (PACE) and since January 2016, has been serving as the Chief Executive Officer.  He sits on the board of The Massachusetts League of Community Health Center, as well as being a board member of Boston HealthNet Corporation and others.  He was also awarded the 2014 Community Service Award from the Massachusetts League of Community Health Centers.

Jay received a Master of Business Administration with a concentration in Health Administration from Suffolk University and a Bachelor of Science in Human Physiology from Boston University.

Paul Geltman, M.D.

Chief Medical Officer

Dr. Paul Geltman has been named Chief Medical Officer of Upham’s Corner Health Center. Geltman comes with a wealth of experience having come from Whittier Street Health Center, Cambridge Health Alliance, and most recently at Franciscan Children’s Hospital as their Ambulatory Care Services Medical Director. He has training in pediatrics and even spent time at UCHC early in his career. He will be a welcome addition to the leadership team and medical staff.

Dr. Henricks is a graduate of the University of Rochester, School of Medicine.  She has been employed at Upham’s Corner Health Center since 1998 and assumed the role of Medical Director in 2013.

She completed a community health center-based residency program in Family Medicine at the Greater Lawrence Family Health Center.  She has spent her entire medical career working in community medicine with diverse populations, including travel to Mexico and Guatemala.

Fluent in Spanish and medical Cape Verdean Creole, Dr. Henricks is committed to providing quality care for vulnerable populations, with a special focus on urban areas.  In addition, she has served as a preceptor for medical students and was awarded the Reach Out and Read Medical Champion of the Year in 2013 for her efforts on behalf of pediatric literacy.

Dan Joo, M.P.H.

Chief Financial Officer/Director of Strategic Planning

Dan Joo worked as the Director of Strategic Planning since 2016.  He was responsible for overseeing Quality and strategic implementation for the organization through the oversight of the Quality Assurance, Process Improvement program and Project Management Office. He recently assumed the position of Chief Financial Officer.

In coordination with Senior Leadership and the Change Management Team, he has overseen the transition into the MassHealth Accountable Care Organization model of care in partnership with Community Care Cooperative, Behavioral Health Community Partners Program, the OCHIN-EPIC electronic health record, and multiple other major initiatives.  His work frequently focused on the development of reporting and analytics systems to inform the decision processes across all levels.

Prior to his current role, Dan worked at UCHC as a Project Manager from 2012 to 2014, and at the Center for Health Information and Analysis between 2014 and 2016.  He received a Bachelor of Arts in Philosophy and Political Science from Rutgers University, and a Master of Public Health in 2011.

Lisa Perrone, LICSW Director of Behavioral Health and Social Services

Lisa Perrone, M.S.W.

Director of Behavioral Health and Social Services

Lisa has been the director of her department for over 10 years.  She oversees behavioral health and social services for the health center. She has been an integral part of integrating the behavioral health department with primary care so patients can be treated holistically improving overall health outcomes for patients.

In addition to her MSW training, she is EMDR trained and her specialty is treating patients who have experienced trauma or those dealing with substance use.  She has been passionate about bringing these effective, evidenced-based treatments to underserved communities.  Lisa prioritizes building a team that fosters multiculturalism and diversity, bringing patient voice and choice to the health care conversations, educating patients and colleagues on the impact of trauma and how we deliver both medical and behavioral health care to patients and enhancing our efforts at UCHC to have all people take on the fight against the stigma around mental health and substance use.

She has been involved in leading initiatives that enhance our center’s capacity to manage patients that demonstrate difficult behaviors and create systems that encourage staff development in de-escalation and other behavioral management techniques that promote safety and security in the workplace.

Nicole Smith

Director, Human Resources

Nicole brings 17 years of human resources experience within community healthcare.  Most recently, Nicole has worked as a Human Resources Director in Joshua Tree, California. Prior to that, she worked for 15 years at MetroWest Medical Center, which consists of two community hospitals located in Framingham and Natick Massachusetts.

Nicole recently joined Upham’s Corner Health Center where she will be responsible for overseeing all human resources operations and employee relations matters.

Nicole attended Northeastern University for Human Resources Management and plans to continue her education for her master’s degree.

Program Director, Upham’s Elder Service Plan

Nancy joined the Upham’s Elder Service Plan in March 2016.  In September 2018, she assumed the role of PACE Program Director.  Her primary responsibilities include oversight and management of PACE clinical and operation staff, ensuring program integrity, and safeguarding the mission of the program.  Nancy works closely with the PACE leadership team to identify strategic and business opportunities.  Together they set the annual goals for the program.

Nancy is also responsible for representing PACE at the UCHC corporate level and is the key contact for both the State and Federal regulators.  Nancy has a thirty five year professional history working in healthcare; her interest and experience has been operational oversight of programs caring for elders and persons with disabilities.  She has a Bachelor of Arts in Economics and Political Science.  In addition, she holds an English as a Second Language degree and certification.

Eileen Segale, R.N.C, M.S., M.P.H.

Director, Home Care Department

Eileen has been the Director of the Home Care Department since 1987.  She participated in securing funding and led the initial home-based Maternal Child Health, HIV, and PACE programs.

Prior to joining UCHC, she held several direct care and leadership positions including Head Nurse, Temple University Hospital; Research Nurse, Columbia University Hospital; MCH Nurse, Boston City Hospital; and Public Health Supervisor, Copan, Honduras (Peace Corps).  Eileen has a Master of Public Health with a concentration in Health Administration and a Master of Science in Nursing, Adult Health, both from Columbia University.

Maryellen oversees the health center operations in collaboration with the entire leadership team to provide efficient, patient-focused, accountable, and compliant processes and systems to support the provision of services to UCHC patients seeking primary, urgent, specialty, including eye and dental care and women’s health at UCHC health centers.

Since coming to the health center, she has developed a centralized call center and led several projects to improve processes impacting patients and staff, such as form re-design process, developing staff career ladders, prescription refill request process improvements, and improved appointment access.

Maryellen has a Master of Business Administration from the University of Massachusetts.  She has over 20 years of professional experience working in health care settings including operations, patient and member services, call center management and compliance at Commonwealth Care Alliance and East Boston Neighborhood Health Center.
