Activities Assistant



Upham’s Elder Service Plan, a division of Upham’s Corner Health Committee, Inc., is looking to hire an Activities Assistant. The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive, continuous health and social services that will support the independence, quality of life, and nurture the spirit of older and disabled adults. The Activities Assistant contributes to the development and implementation of individual and group therapeutic recreation programs at our Adult Day Health Centers.

Behavioral Health Integrated Counselor


We are seeking for a Behavioral Health Clinician to serve as an integrated counselor and advocate for patients coping with mental health and substance use disorders by collaborating with providers to enhance Primary Care and Behavioral Health integration. The goal of the position is to help empower patients to understand decisions made about their care and prescribing while also providing empathy for those difficulties. This position will be central in Upham’s Corner Health Center’s role to address the substance use epidemic in the community by addressing stigma and engaging individuals in treatment. This position will be integrated into the primary care departments to provide clinical behavioral health consultation to providers and staff as requested.

Behavioral Health Navigator


We are seeking for a Behavioral Health Navigator to serve as a specialized Community Health advocate and case manager by addressing educational and psycho-social needs of patients seeking services for substance use and behavioral health. This position will work to provide prevention activities, support, and case management to individuals in need of additional support to meet their care plan goals. These services will promote positive protective factors, focus on positive social and emotional development, reduce traumatic stressors and triggers, and support healing. The goal for the Behavioral Health Navigator is to help to improve outcomes and engagement in treatment with high-risk individuals through connection to services and engagement on patient-focused educational events.

Center Manager – Program of All-Inclusive Care for the Elderly (PACE)



Upham’s Elder Service Plan, a division of Upham’s Corner Health Committee, Inc., is seeking for a Center Manager. The Program of All-Inclusive Care for the Elderly (PACE) is an innovative model that provides all-inclusive integrative primary care and social services to older adults.

The Center Manager manages and coordinates the daily activities of an Adult Day Health Center Program by providing overall supervision, training, competency review, performance evaluations and management for health center aides and activity staff. Health Educator

The Health Educator develops and implement individual and group health education and awareness of health maintenance goals and facilitate navigation to care to ensure reaching health maintenance targets e.g. colon cancer screening, STI screening, well-child exam goals, smoking cessation etc.


The goals of the Health Educator and the Health Education Program are to improve patient access to health maintenance testing and screenings as well as child visits, to provide culturally and linguistically appropriate comprehensive health education services, to increase patient understanding of the procedures/screenings, or well-child visits and their importance, and to increase completion of the procedures/screenings or well-child visits.


The primary objectives of the health education intervention are to decrease missed or canceled appointments and to promote well-child visits or early detection in the relevant populations. These two interventions in patient education and support, and helping to navigate the scheduling system, will help patients better understand and reduce their concern about the procedure/screening or reduce poor show rates for well-child visits.


Responsibilities will include attending training, conducting health education and counseling, documenting services providing in EMR and completing quality assurance projects and program reports. Additional responsibilities include Medical Interpreting.

Psychiatrist / Psychiatric CNP Prescriber



The Psychiatrist / Psychiatric CNP provides consultation and support to the clinical team regarding mental health/substance abuse diagnosis and treatment, psychopharmacology consultation and education. The position provides mental health and addiction treatment at the request of the primary care provider. The position coordinates mental health/addiction treatment services in accordance with the treatment plan, and works collaboratively with the clinical team to manage or co-manages complex mental health cases.

This position is responsible for providing psychopharmacology services for the Behavioral Health department clients. The position is responsible for establishing all behavioral health medical policies and protocols in collaboration with the Director of Behavioral Health and for supervising all psychiatric related services provided. The position functions under the general supervision of the Director of Behavioral Health.
