We are seeking a detail-oriented and mission driven Chief Financial Officer who will be responsible for the leadership, direction and administration of all financial aspects of Upham’s Corner Health Committee. Upham’s Corner Health Center is currently seeking a Director of the Home Health Aide Services Department who will be responsible for the oversight and administration of the departmental budget.

E.H.R. Site Specialist



Reporting to and working closely with the Director of Medical Informatics, the OCHIN/Epic Site Specialist will provide daily support to staff that uses OCHIN. The Site Specialist will also develop and provide OCHIN training and communicate system updates to end users. Finally, the Site Specialist will be considered the subject matter expert for OCHIN, and be asked to provide input on clinical and operational workflow changes involving the EHR. Home Health Aide Service Supervisor



Under the direction of the Home Health Aide Manager, the Home Health Aide Services Supervisor is responsible for overseeing tasks done by the administrative scheduling assistants (scheduling, evaluations, and preparing timesheets) and assisting with supervising home health aides. .In addition, he HHAS Supervisor works collaboratively with the HHAS Manager on oversight of the daily operations of the department.

PACE Home Care Coordinator



The Home Care Coordinator (HCC) is a member of the PACE Interdisciplinary Team (IDT) and collaborates closely with the UCHC Home Health Aide Services (HHAS) department and with contract providers to assure home-based services are provided to PACE participants in accordance with the mission of PACE and in adherence to the participant’s individualized care plan. The HCC works in close collaboration and coordination with PACE Primary Care, Rehabilitation and Nursing departments to monitor that HHA services are provided. The HCC acts as the liaison between PACE and the HHAS department, contracted home-based service providers, and nursing facilities, assuring timely and integrated service delivery. The HCC oversees and directly provides training, supervision, and competency evaluation for Personal Care Workers.

Primary Care Physician–Program of All-Inclusive Care for the Elderly (PACE)



We are seeking for a Primary Care Physician to work in an innovative, community-based model of care for older adults. Psychiatrist / Psychiatric CNP Prescriber



The Psychiatrist / Psychiatric CNP provides consultation and support to the clinical team regarding mental health/substance abuse diagnosis and treatment, psychopharmacology consultation and education. The position provides mental health and addiction treatment at the request of the primary care provider. The position coordinates mental health/addiction treatment services in accordance with the treatment plan, and works collaboratively with the clinical team to manage or co-manages complex mental health cases.

This position is responsible for providing psychopharmacology services for the Behavioral Health department clients. The position is responsible for establishing all behavioral health medical policies and protocols in collaboration with the Director of Behavioral Health and for supervising all psychiatric related services provided. Social Worker – Program of All-Inclusive Care for the Elderly (PACE)



Upham’s Elder Service Plan, a division of Upham’s Corner Health Committee, Inc. (UCHC) is currently seeking a dedicated and detail-oriented Social Worker. The individual in this role provides initial psychosocial assessments, continuing re-assessments, and social work interventions for program participants to promote their social, psychological, spiritual, and physical well-being. The Social Worker works in close collaboration with all Plan of All-Inclusive Care for the Elderly (PACE) team members, community and family caregivers.
