Administrative Assistant for Upham’s Elder Service Plan (PACE)


The Administrative Assistant position works in collaboration with the Operations Director and Lead Administrative Assistant to streamline back office processes and ensures accuracy and quality in the work performed. This role is assigned individual responsibility for specific priority and completion of ongoing tasks.

Care Coordinator



The Care Coordinator will serve as a member of Upham’s Corner Health Center’s Behavioral Health Community Partners (BH CP) team, providing person-centered and recovery-oriented care management and coordination to assigned Enrollees.


This position was developed as part of the Riverside Community Partners program initiative. The Riverside Community Partners program is a Behavioral Health Community Partners (BH CP) program made up of Riverside Community Care as lead contractor to Mass Health and six (6) affiliated partner organizations, one of which is Upham’s Corner Health Center. The partnership supports functions focused on Care Management and Care Coordination to assist members with high behavioral health needs (SMI and/or SUD) in better navigating the complex behavioral health system.

Dentist



The Dentist provides dental care for patients by diagnosing and treating diseases, injuries, and malformations of teeth and gums as well as performs specialized treatment such as orthodontics.

Enrollment, Reconciliation, Retention Specialist


The Enrollment Reconciliation Retention Specialist documents and conducts procedures related to the reconciliation of Medicaid, Medicare, and Part D enrollment membership and corresponding premium. The Enrollment Reconciliation Retention Specialist works directly with participants and their families to obtain documents for Mass Health Eligibility and submits Mass Health applications on behalf of the participant.

The Enrollment Reconciliation Retention Specialist submits enrollment data and reviews regularly received reports from the Center for Medicare and Medicaid Services (CMS), Massachusetts Medicaid and Upham’s Elder Service Plan (UESP) contractors to identify and resolve data discrepancies.
