Associate Medical Director, Chief NP/PA



Upham’s Corner Health Center, a division of Upham’s Corner Health Committee, Inc. (UCHC) is currently seeking an Associate Medical Director. The Associate Medical Director/ Chief NP/PA will collaborate with the Medical Director to provide direction and momentum for the health center. This Advanced Practice Clinician will provide clinical leadership through the development and implementation of new health center initiatives and help to optimize current functions.

Behavioral Health Integrated Clinician – Women’s Health



Upham’s Corner Health Center, a division of Upham’s Corner Health Committee, Inc. Benefits Assistant


We are currently seeking a detail oriented and bi-lingual Benefits Assistant. The ideal candidate for this position must be fluent in English and Spanish, Portuguese and/or Cape Verdean Creole as this is a strict requirement for this role. The Benefits Assistant assists our patients who do not have Health Insurance coverage and the underinsured to enroll in appropriate health programs to cover their health care costs and prescription medication needs.

Care Coordinator



The Care Coordinator will serve as a member of Upham’s Corner Health Center’s Behavioral Health Community Partners (BH CP) team, providing person-centered and recovery-oriented care management and coordination to assigned Enrollees.


This position was developed as part of the Riverside Community Partners program initiative. The Riverside Community Partners program is a Behavioral Health Community Partners (BH CP) program made up of Riverside Community Care as lead contractor to Mass Health and six (6) affiliated partner organizations, one of which is Upham’s Corner Health Center. The partnership supports functions focused on Care Management and Care Coordination to assist members with high behavioral health needs (SMI and/or SUD) in better navigating the complex behavioral health system.

Community Health Advocate

Upham’s Elder Service Plan, a division of Upham’s Corner Health Committee, Inc. Credentialing/Provider Enrollment Specialist



Upham’s Corner Health Center, a division of Upham’s Corner Health Committee, Inc. (UCHC) is currently seeking for a self-starter and detail-oriented Credentialing/Provider Enrollment Specialist. The person in this position is responsible for all aspects of the provider credentialing and insurance plan participation/enrollment for providers that submit claims to insurance payers. Director, Home Health Aide Services



Upham’s Corner Health Center is currently seeking a Director of the Home Health Services Department who will be responsible for the oversight and administration of a $5 million departmental budget.


RESPONSIBILITIES:


The Director of Home Health Services works closely with the Upham’s Home Care Department, Upham’s Elder Service Plan (ESP) and contracted vendors to ensure compliance to regulatory requirements and provide fiscal oversight. The individual in this role ensures departmental training meets compliance and policy requirements.



The Director of Home Health Services is also responsible to identify department /program needs, including but not limited to the hiring of Health Aides and monitoring work activities completed by Administrative Scheduling Assistants. The Director will perform other duties as assigned by supervisor. Farmer’s Market Manager/Part time)
10 hours per week


Upham’s Corner Health Center, a division of Upham’s Corner Health Committee, Inc. (UCHC) is currently seeking for a charismatic individual who is interested in agriculture, food, and community to manage the first ever Upham’s Corner Farmers Market. The Market Manager is the go-to person for community partners, farmers, producers, and shoppers during the market season. This market is part of the Imagine Boston 2020 vision being implemented by the Mayor’s Office and other community partners.


The Upham’s Corner Farmers Market is held every Wednesday starting July 18th from 3:00 pm to 7:00 pm. The location of the market will be at 559 Columbia Rd, Dorchester, Massachusetts. Market Managers must be able to commit to a full market season which runs from July 18th,2018 through October 24th,2018. Hours are approximately 6 hours per market day and 4 hours of administrative/budget work per week. The rate of pay is $20 per hour.
