Care Coordinator



The Care Coordinator will serve as a member of UCHC’s Behavioral Health Community Partners (BH CP) team, providing person-centered and recovery-oriented care management and coordination to assigned Enrollees, including but not limited to: outreach and engagement, assisting Enrollees to identify goals and to develop an individual care plan, completing comprehensive Assessments, facilitating Care Teams, coordinating communication among Enrollees, their healthcare and other providers, involved state agencies and others, and supporting Enrollees to access and utilize resources to improve and maintain their health and quality of life.

Community Health Advocate Supervisor



The Senior Community Health Advocate (CHA) oversees the operations and functions of the Community Health Advocate staff. The Senior CHA organizes, and coordinates the services being provided by the Community Health Advocates and outreach services.

E.H.R. Site Specialist



Reporting to and working closely with the Director of Medical Informatics, the OCHIN/Epic Site Specialist will provide daily support to staff that uses OCHIN. The Site Specialist will also develop and provide OCHIN training and communicate system updates to end users. Finally, the Site Specialist will be considered the subject matter expert for OCHIN, and be asked to provide input on clinical and operational workflow changes involving the EHR. The Site Specialist will provide emergency support with Patient Access issues (e.g., e-prescribing, orders, scheduling).

Human Resource Generalistt



The Human Resource Generalist is a key member of the HR team and is responsible for performing HR-related duties on a professional level and works closely with the HR Director in supporting the needs of Upham’s Corner Health Center. Human Resource Specialist



The Human Resources Specialist is a key member of the HR team and closely partners with the HR Generalist and HR Director. Lead Pediatrician


The Lead Pediatrician collaborates with the Medical Director, the Primary Care Associate Medical Director, and other UCHC leadership to optimize care for patients up to age 21.

Responsible for all Marketing and Promotions efforts for Upham’s Elder Service Plan (PACE).     Psychiatrist / Psychiatric CNP Prescriber



    The Psychiatrist / Psychiatric CNP provides consultation and support to the clinical team regarding mental health/substance abuse diagnosis and treatment, psychopharmacology consultation and education. The position provides mental health and addiction treatment at the request of the primary care provider. The position coordinates mental health/addiction treatment services in accordance with the treatment plan, and works collaboratively with the clinical team to manage or co-manages complex mental health cases.

    This position is responsible for providing psychopharmacology services for the Behavioral Health department clients. The position is responsible for establishing all behavioral health medical policies and protocols in collaboration with the Director of Behavioral Health and for supervising all psychiatric related services provided. The position functions under the general supervision of the Director of Behavioral Health.
