Behavioral Health Integrated Counselor


We are seeking for a Behavioral Health Clinician to serve as an integrated counselor and advocate for patients coping with mental health and substance use disorders by collaborating with providers to enhance Primary Care and Behavioral Health integration. The goal of the position is to help empower patients to understand decisions made about their care and prescribing while also providing empathy for those difficulties. This position will be central in Upham’s Corner Health Center’s role to address the substance use epidemic in the community by addressing stigma and engaging individuals in treatment. Behavioral Health Navigator


We are seeking for a Behavioral Health Navigator to serve as a specialized Community Health advocate and case manager by addressing educational and psycho-social needs of patients seeking services for substance use and behavioral health. This position will work to provide prevention activities, support, and case management to individuals in need of additional support to meet their care plan goals. These services will promote positive protective factors, focus on positive social and emotional development, reduce traumatic stressors and triggers, and support healing. Director of Medical Informatics

We are currently seeking a Director of Medical Informatics to lead our Electronic Medical Records (EMR), workflow optimization efforts and clinical employee EMR training. The Director of Medical Informatics supports the organization’s Quality Assurance, reporting, and billing teams to improve quality of care and maximize revenue. This position will serve as the key clinical liaison internally and externally for electronic systems impacting clinical processes and the Electronic Health Records (EHR) with respect to clinical workflow analysis.

Enrollment, Reconciliation, Retention Specialist


The Enrollment Reconciliation Retention Specialist documents and conducts procedures related to the reconciliation of Medicaid, Medicare, and Part D enrollment membership and corresponding premium. The Enrollment Reconciliation Retention Specialist works directly with participants and their families to obtain documents for Mass Health Eligibility and submits Mass Health applications on behalf of the participant.

The Enrollment Reconciliation Retention Specialist submits enrollment data and reviews regularly received reports from the Center for Medicare and Medicaid Services (CMS), Massachusetts Medicaid and Upham’s Elder Service Plan (UESP) contractors to identify and resolve data discrepancies.

Health Educator

The Health Educator develops and implement individual and group health education and awareness of health maintenance goals and facilitate navigation to care to ensure reaching health maintenance targets e.g. colon cancer screening, STI screening, well-child exam goals, smoking cessation etc.


The goals of the Health Educator and the Health Education Program are to improve patient access to health maintenance testing and screenings as well as child visits, to provide culturally and linguistically appropriate comprehensive health education services, to increase patient understanding of the procedures/screenings, or well-child visits and their importance, and to increase completion of the procedures/screenings or well-child visits.


The primary objectives of the health education intervention are to decrease missed or canceled appointments and to promote well-child visits or early detection in the relevant populations. These two interventions in patient education and support, and helping to navigate the scheduling system, will help patients better understand and reduce their concern about the procedure/screening or reduce poor show rates for well-child visits.


Responsibilities will include attending training, conducting health education and counseling, documenting services providing in EMR and completing quality assurance projects and program reports. Additional responsibilities include Medical Interpreting.

The Psychiatrist / Psychiatric CNP provides consultation and support to the clinical team regarding mental health/substance abuse diagnosis and treatment, psychopharmacology consultation and education. The position provides mental health and addiction treatment at the request of the primary care provider. The position coordinates mental health/addiction treatment services in accordance with the treatment plan, and works collaboratively with the clinical team to manage or co-manages complex mental health cases.
